Lyn Hernandez

Lyn Hernandez, PMHNP-BC, is a psychiatric nurse practitioner serving patients in National City through Pacific Health Systems. She provides compassionate, individualized mental health care and is committed to helping each client feel heard, supported, and confident as they move through treatment.

Lyn is trained in Spravato and TMS, giving her a strong understanding of the unique needs of patients receiving interventional psychiatric therapies for depression and other mood-related conditions. She works closely with clients to provide thoughtful evaluations, personalized treatment planning, and ongoing support throughout their care journey.

In addition to outpatient services, Lyn also cares for patients in a variety of community-based and residential settings, including boarding care homes, independent living facilities (ILFs), and skilled nursing facilities (SNFs) throughout San Diego County. This experience allows her to support individuals with complex psychiatric and behavioral health needs across multiple levels of care.

She proudly accepts most major insurance plans, including TRICARE, TriWest, Medicare, and Medi-Cal, helping make quality mental health care more accessible. Lyn is currently accepting new clients in National City and is dedicated to providing warm, respectful, and patient-centered care to the community.
